Community Support:
Community based programming provides a range of opportunities to learn activities of daily living, and increase independence and socialization.
Homemaker/Companion Services:
Our trained caregivers provide light housekeeping, meal preparation, run errands, or accompany individuals in the community. We provide quality, caring companionship to the elderly and adults with disabilities.
Social Skills:
Facilitation of social skills training supported by evidence-based research that promotes the development of interpersonal and communication skills necessary for successful interactions within home and community.
Employment Assistance:
Employment assistance consists of services and supports provided to assist individuals in locating paid employment in the community. The service helps identify employment preferences; job skills; requirements for the work setting, work conditions, and prospective employers that may offer employment opportunities compatible with the individual’s identified preferences, skills, and requirements.
Supported Employment:
The supported employment service component provides ongoing individualized support, needed by an individual to sustain paid work in an integrated work setting.
Case Management and Service Coordination:
A service coordinator will assess, plan and manage your person-centered service plan to help improve your quality of life and create more pathways to independence.
Personal Support Networks:
A trained PSN facilitator will assist families in vetting and training carefully selected family, friends, and professionals in their roles as part of a personal support network. These individuals will provide long-term companionship and assistance to keep individuals connected to the community.
